ARDS is a heterogenous syndrome with high mortality rate and limited therapeutic strategies. PEEP routinely applied to these patients as part of protective lung ventilation strategy because getting it right helps optimize respiratory system compliance improve oxygenation. However, if the too low or high, may cause harm. Large clinical trials, focusing on ARDS, have generally compared strategies designed deliver those higher PEEP. No superior has emerged, perhaps studies did not stratify based their likely response Nevertheless, best identify optimal remains controversial, despite many large over past 50 years. In modern era personalized medicine, our should be but individualized this manuscript, we review physiological effects various methods studied available determine at bedside, providing guide for physicians who regularly manage mechanically ventilated highlighting new avenues research.
